When to use

Derive LDL when not directly measured.

How it works

LDL = Total cholesterol − HDL − Triglycerides/5 (mg/dL). Invalid when TG > 400 mg/dL.

Key points

  • Underestimates LDL at high TG and very low LDL.
  • Use directly measured LDL if TG > 400 or type III dyslipidaemia.
  • Treatment goals depend on overall cardiovascular risk.
