When to use

Estimate equivalent doses when switching agents or substituting long-acting diazepam for short-acting drugs during a taper.

How it works

Diazepam equivalent = dose × (10 / agent equivalence); target = diazepam-eq × (target equivalence / 10). Ashton: diazepam 10 = alprazolam 0.5 = lorazepam 1 = clonazepam 0.5 = oxazepam 20 mg.

  • Equivalences differ widely between references — treat as an estimate only.
  • When tapering, long-acting diazepam smooths withdrawal from short-acting agents.
  • In hepatic impairment prefer oxazepam/lorazepam (not oxidatively metabolised).
